Family Carnival Participation Waiver & Release of Liability

By checking the box below and allowing my child(ren) to participate in activities at the St. Peter Lutheran Church and St. Peter Lutheran Preschool Family Carnival, I acknowledge and agree to the following:

  1. I understand that participation in carnival activities, including but not limited to inflatable attractions operated by Just Add Kidz (Jumpin' JAKZ) and animal exhibits provided by Ford Hoof & Horns, involves inherent risks, including but not limited to falls, collisions, slips, bites, scratches, kicks, allergic reactions, and other injuries.

  2. I voluntarily assume all risks associated with my child(ren)'s participation in these activities and understand that accidents may occur despite reasonable safety precautions.

  3. I certify that my child(ren) are physically able to participate and that I will not allow participation if they have a medical condition, allergy, or other circumstance that could make participation unsafe.

  4. I understand that I am responsible for supervising my child(ren) at all times and for ensuring they follow all posted rules and instructions provided by event staff, volunteers, or activity operators.

  5. In consideration for allowing my child(ren) to participate, I release, waive, and discharge St. Peter Lutheran Church, St. Peter Lutheran Preschool, their pastors, employees, volunteers, officers, directors, and event organizers from any and all claims, demands, causes of action, damages, losses, or liabilities arising out of or related to my child(ren)'s participation in carnival activities, except to the extent caused by gross negligence or willful misconduct where such liability cannot legally be waived.

  6. I understand that Just Add Kidz (Jumpin' JAKZ) and Ford Hoof & Horns are independent vendors responsible for the operation of their own attractions, equipment, animals, and activities.

  7. I understand that interactions with animals are voluntary and that animals, even when properly handled, may behave unpredictably. I accept the risks associated with being near or interacting with live animals.

  8. I grant permission for emergency medical treatment to be obtained for my child(ren) if I cannot be reached immediately. I understand that I am responsible for any resulting medical expenses.. I understand that I am responsible for any resulting medical expenses.
